You can use this page to upload your Standard Financial Statement (SFS) and the required supporting documents.

Complete the standard financial statement in one of the following ways:

By phone

By post

Send the filled SFS form to

  • If you complete your SFS online, we will phone you to complete voice authorisation, which substitutes the need to sign the form. This will happen within 48 hours.
  • If you send a signed form through this page (using Docuload), you will still receive a phone call to confirm your voice authorisation. We cannot progress your case until we do this.
  • Please upload all supporting documents through this https://www.havenmortgages.ie/docuload or alternatively post them to us at the address above.
  • We'll assess your SFS when we are happy it’s completed properly, have your voice authorisation and all supporting documents.

    The information provided in the Standard Financial Statement (SFS) and supporting documentation is vital in helping Haven understand customer’s financial situation. This statement identifies your current income, your living expenses plus other expenditure and financial commitments that you may have.

    You need to provide these documents with your SFS:

    Most recent Self-Assessment Certificate or Notice of Assessment

    The most recent social welfare receipt for each spcial welfare payment received

    *If the social welfare payment is mandated to the current accunt for which statements are being provided, social welfare receipt are not required

    • All Supporting Documents must be dated within the past 3 months.
    • Internet statements must clearly show your name and address.
    • If you are taking a photo of your documents, please ensure all edges of the document are within the camera frame.

    Assessment of your SFS cannot begin until we receive all the documents we asked for.
